Online Registration
You can now register through the NHS website.
Paper Registration
Where possible, please provide a photo ID (e.g. passport or driving licence) and proof of address, . You will also need to complete two forms: a NHS registration form and a new patient registration pack. The pack consists of a health questionnaire, consent for sharing medical records and an application for access to online services. Forms are needed for every patient who wishes to register, including children. All forms are available from Reception or to download via the links below.
Registration Questionnaire (NPHQ)
Once we have your registration forms, it can take up to a week to complete the full registration process before you can book routine appointments. If you need to see a GP for a non-urgent issue in this period, please visit any of the local walk-in centres. Details of the nearest centres are on our website under: Emergencies / Medical Assistance Out of Hours.
We may not receive your medical notes from your previous doctor for several weeks. We recommend that you order repeat prescriptions from your current doctors before registering with us if you are running low on medication. If you need a repeat prescription urgently, please bring a prescription tear off slip or letter from your previous doctor or hospital.
Non-English speakers
There are translated questionnaires available from Doctors Of The World. They offer versions in 28 languages, for adults and children.

Migrants, Refugees & Asylum Seekers, Unhoused and Undocumented People
You can still register, even if you don’t have proof of ID or Address.
We are a Safe Surgery, and your information is confidential. Please speak to us if you are worried about providing an address or other details.
If you do not have an address, you can still register with a correspondence address. Please speak to us about the best options for you.
Temporary Registration
If you are ill while away from home, or if you are not registered with a doctor but need to see one, you can receive emergency treatment from a local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
